[MASTA] MASTA Chamber Music Registration Form

Dear Chamber Music Enthusiasts,

If you plan on registering a group for the MASTA Chamber Music Festival, 
please use the attached pdf form. This is a fillable form that should 
(in a perfect technological universe) save me time when processing 
registrations. Here are the instructions:

1. Download the form to your computer

2. Complete the form in Adobe Acrobat Reader

3. Save the completed form to your computer using the "Save As..." 
option in the File menu

4. Email the form to me as an attachment.

Please forward this email to anyone you know who plans to register a 
group for the festival.

Please register by Feb. 11.
